NSW Health has confirmed four children have been issued with incorrect test results due to a human error at Westmead Children’s Hospital.

The was confirmed when testing on a newborn baby failed to match earlier prenatal test results for chromosomal abnormalities.

Dr Kerry Chant, the state’s Chief Medical Officer, tells Mark Levy the “isolated incident” occurred in the genetics lab.

“We are committed to understanding how this occurred and are putting all safeguards in place so we prevent it happening again.”

Dr Chant says the matter is being taken “very seriously”.

“This is the first time incorrect results have been issued. We’ve commenced an investigation and an independent review.

“I would like to extend my sincere apologies to those patients affected by this. ”
